Getting Back on Your Feet
Illness brings with it many anxieties and concerns that can be discussed with you in confidence. Our Social Workers can assist you and your family with counselling, stress management and coping strategies. The services provided by our Discharge Coordinators, Social Workers, and Case Managers form part of our care for your recovery.
There are also many services and options available to help you maintain independence and a good quality of life in times of illness. Our Discharge Coordinators, Social Workers and Case Managers can provide practical assistance in the areas of:
- Home services to help with personal care, meals, housekeeping and transport, for example
- Resources to assist when illness or incapacity creates financial problems
- Convalescent and nursing care
- Information on community and self-help support groups
